ADMISSION PROCEDURES
 
To Nursing Home:-
  1. Family seeks assistance of Medical Social Worker (MSW) at the Hospital or Polyclinic where the patient is receiving or used to receive treatment for placement in a nursing home.
     
  2. MSW of the Hospital or Polyclinic submits the application form to the Integrated Care Service (ICS) for placement.
     
  3. ICS forwards the application form to the selected nursing home for intake assessment.
     
  4. Upon receiving the application form, our nursing staff would visit to assess the patient.
     
  5. For all accepted referrals, the MSW of the nursing home will contact family members and arrange for pre-admission interview, financial counselling, etc.
To Hospice:-
  1. Family seeks assistance of the doctors/ Medical Social Worker (MSW) at the hospital where the patient is warded or the home care staff nurse in making the referral.
     
  2. The referral person forwards the application form to the hospice for intake assessment.
     
  3. Upon receiving the application form, our nursing staff would visit to assess the patient.
     
  4. For all accepted referrals, the MSW of the nursing home will contact family members and arrange for pre-admission interview, financial counselling, etc.
